Can you run any other form?
Try to recompile the .fmb file.
Does OEM show that all the services are up?
Try using stopping the forms service at the unix prompt and starting it back up from unix.

i have only one application that runs on forms and it is down.

In OEM, Forms service is down and it is disabled for selection. But when i click on the service, the status is UP. its a strange thing i am observing.

i start all the services using scripts.
